Where do I get the following to transfer hosts?

  I’m trying to change hosts and they are requesting the following info:
  FTP Address
  FTP Username & Password
  Database Control Panel URL (and user & password)
  Registrat’s Control Panel URL (and user & password)

  Where do I find this info or can you supply me with it please?

  The blog I need help with is: (visible only to logged in users)

  Please note that FTP access is not allowed on WordPress.com as seen here:

  http://en.support.wordpress.com/ftp-access/

  If you would like to change hosts, all you need to do is update your nameservers to the new host.
