Which contact form is this?

  I installed a contact form and very easily modified link here http://simonsmountaininstructoraward.wordpress.com/booking-form/
  I want to use the same contact form again but cannot see anywhere that tells me which pluggin it is. Most frustrating. I did the job once and totally failing to even identify the product. please help.
