WP-Script Error

I am seeking assitance with two errors that are displaying on the public wesbite, visible to all visitors. I’ve pasted the errors below for reference. I look forward to your reply and assitance.

The site I need help with is http://www.ddinterventions.org/. It is not hosted by WordPress.com or connected with Jetpack. This site is not linked to my WordPress.com account.
